`.mcp.json` is per-machine MCP server config (e.g. the GitHub MCP block
added during today's session). It references local env vars for auth
rather than embedding secrets, but the file itself is workstation-specific
— what servers a contributor connects depends on which MCPs they've set
up locally.

Per gstack team-mode install: adds a PreToolUse hook that blocks
skill usage when gstack isn't installed globally, so contributors
are prompted to install it. Un-ignores the two required files
(.claude/settings.json, .claude/hooks/check-gstack.sh) while
keeping settings.local.json and other Claude state ignored.

- Show previously-created custom steps as clickable options on decision
nodes so they remain accessible after going back
- Fix breadcrumb to show custom step titles instead of raw UUIDs
- Fix ContinuationModal to show grandchildren (two levels deep) instead
of immediate children that duplicate option labels
- Remove redundant "Save to Library" checkbox from StepForm since
PostStepActionModal now handles that decision
