feat: add resolution output API endpoints

Adds GET /outputs, PATCH /outputs/{id}, and POST /outputs/{id}/push
endpoints under /ai-sessions/{session_id}/, plus integration tests.
Router registered before ai_sessions to avoid path conflict.
